WHY WE COLLECT INFORMATION ABOUT YOU
TOA Clinic hold clinical information about you to ensure practitioners have a complete and continuous record about your past, current and future treatment. Your records may be written down, or held on a computer.
To identify who you are we need your:
Name
Address
Date of birth
Please note we may need your NHS number in some cases.
Information is held solely for the purpose of your health and wellbeing and will only be shared with practitioners and agencies involved in your treatment and care.
HOW WE KEEP YOUR INFORMATION CONFIDENTIAL
All TOA staff members have a duty of confidentiality.
We store your files appropriately and only relevant members of staff have access to your records.
The organisation actively implements and regularly audits security measures, to ensure your information is safe.
We have a strict Subject Access Request policy and your information will only be provided to you when the TOA Information Governance team are satisfied of your identity.
HOW TO ACCESS YOUR RECORDS
You can request access to your information in writing or you can ask to see your records whilst in a consultation with your practitioner, they will then explain what they have written.
When a formal request has been received in writing, the relevant person will:
Forward the written request to the TOA Clinic’s Information Governance Team
Forward proof of your identity to the team, i.e. a utility bill (within the last three months) and a photographic ID
A charge of £10 may be requested, depending on the size and complexity of the request. The fee must be paid prior to you receiving the information
The request can take up to 40 days to be processed
If you have agreed we will use your data to send you email and text messages for marketing and promotional reasons.
WHAT ELSE CAN YOUR INFORMATION BE USED FOR?
Your information can be used for clinical audit following or during a consultation or treatment Clinical Audits review current standards of patient care against acceptable best practice. Audit results are discussed by practitioners to make improvements to patient care. Audit results can be published and/or presented, but always in an anonymised format so that individuals cannot be identified.
Your information could be used for research. Research seeks to investigate new treatments, interventions and management procedures so that treatment outcomes are continually improved. Clinical Audits monitor what has been done. Researchers look at what could be done in the future.
Your information may be used to help manage and plan services. It may be used in an anonymised format for measurement such as waiting times, quality of information and care, and information given about treatment.

IF YOU FAIL TO PROVIDE PERSONAL DATA
Where we need to collect personal data by law or under the terms of a contract we have with you and you fail to provide that data when requested, we may not be able to perform the contract we have or are trying to enter into with you (for example, to provide you with goods or services). The TOA Clinic website logs IP addresses and browser types to provide you with the best possible user experience and allow us insight into consumer behaviour. You can browse our site without entering any personal information, and therefore remain anonymous during your visit. However, the enquiry form asks for personal information such as name, postal address, email address and telephone number to enable us to send out the requested information, and make any future visits to our site easier.

NO FEE USUALLY REQUIRED
